: All protocols have been carried out in accordance with UK National Health Service Health Research Authority (HRA) regulations. Ethical approval was provided by the national Health Research Authority (Ref. 16/HRA/5495). After receiving approval by the HRA, researchers applied for and were granted a National Health Service Research Passport, granting them access to the Trust to conduct interviews and observations. Written, informed consent for participation was obtained from all participants in the study.
