: Ethical approval was received from the Regional ethical review board in Gothenburg, Sweden (No. 931–15). The study was performed according to the Declaration of Helsinki and national ethical regulations (ExternalRef removed) [CitationRef removed]. Written informed consent was retrieved from all participating adolescents and parents in the RCT after they had received written and verbal information about the study. Participating adolescents in the qualitative data collection received additional written and verbal information about the qualitive study, along with retrieval of written informed consent. According to Swedish regulations, ethical approval is not required for interviews performed in samples comprising of healthcare professionals when no sensitive personal data is obtained (ExternalRef removed). However, as the study was performed according to the Declaration of Helsinki [CitationRef removed] information about the study purpose and methodology was given in person and all participating HCPs and representatives from the patient organization consented verbally to participation. Although an ethical permit was not necessary for interviews with HCPs, this was included in the ethical application and approved. The consent was documented by the first author (MS) responsible for the data collection at the time of the interview, focus group discussion and/or observation.
