: The study is based on secondary use of Hospital Episodes Statistics, which are data collected by the National Health Services (NHS) in England for the purposes of routine care. The use of Hospital Episodes Statistics data was approved by NHS Digital (Health and Social Care Information Centre) for the purpose of this study (DARS-NIC-393510-D6H1D-v6.11) and data provided to us were de-identified. Access to data were granted through a Data Access Request Service application through NHS Digital and approved following review by the Independent Group Advising on the Release of Data (IGARD); IGARD considered that informed consent was not required as data provided to researchers were not identifiable.
