: The Health Research Authority (HRA) confirmed survey A was not considered to be research and did not require review by an NHS Research Ethics Committee. Nevertheless, survey A was registered as a service evaluation with NHS Grampian (Project ID 4679). The design of Survey B was approved by the University of Aberdeen School of Medicine, Medical Sciences and Nutrition Ethics Review Board (CERB/2019/11/1805).
