The Norwegian Data Protection Authority approved the use of the data for research purposes in this project (17/01372–2/SBO). The register owners, Statistics Norway, the Norwegian Directorate of Health, and the Norwegian Institute of Public Health, approved linkage of registries. The data were pseudo-anonymized by third party (Statistics Norway) and analysed at group level to minimize the risk for backwards identification of individuals.
