: This study received the required ethics approval from Tehran University of Medical Sciences Research Ethics Committee, Tehran, Iran, with ethics code No. IR.TUMS.MEDICINE.REC.1399.983. In this study no data on human subjects nor human material have been recorded or analyzed. This means that this study does not fall under the heading of ‘medical research’ as stated in the Declaration of Helsinki, nor do any of the principles mentioned in this Declaration apply to our research. Data related to NHA were collected from the National Statistics Portal from 2014 to 2018. Cost and referral rates data, which were related to the main BIOs, were based on the statistical yearbooks uploaded on Iran Health Insurance Organization (IHIO) and Social Security Organization (SSO) websites. It should be noted that access to this data was allowed for everyone and there was no need for a special license.
