: Management approval was obtained from NHS Fife. The study protocol was reviewed and approved by internal staff in NHS Fife and by one external reviewer from NHS Forth Valley. A formal review by an NHS ethics committee was not required following advice obtained using the Health Research Authority decision tools (ExternalRef removed) and directly from the East of Scotland Research Ethics Service (NHS Tayside).
: Completion of the questionnaire was taken as implied consent. Informed consent was obtained from all subjects who volunteered to take part in the qualitative interview arm of the study. All study methods were carried out in accordance with the principles written into the Declaration of Helsinki (June 1964).
