Written informed consent was obtained from each participant. Informed consent was obtained from all individual participants included in the study. The interviewer obtained informed consent by a written signature from participants (or their parent or legal guardian in the case of illiterate participants) prior to each interview. To ensure data confidentiality and anonymity, data were anonymized and irreversibly de-identified to protect patients, healthcare providers, hospital and community privacy.
