Ethical approval was obtained from the University of New South Wales Human Research Ethics committee (approvals HC210308 and HC210560). Informed consent to use newsletter subscribers’ details for the purposes of this study (e.g. survey invitation) was obtained at the point of subscription (Participant Information Statement presented during the subscription process). Written informed consent was obtained from each interview participant at the start of their interview (Participant Information Statement presented prior to the interview), and submission of survey by each participant was taken as an indication of their consent (Participant Information Statement presented at the start of the survey).
