: Ethical approval was obtained from the Ethics Review Committee of Kenyatta University (KU/R/COMM/51/411) and the Ethics Committee of Bonn University (246/14). Free and informed written consent of the study participants was obtained for participation in the study. Information about the study, potential harms and benefits, the purpose of the study and the right to withdraw were emphasized, both in the written material and verbally to the study participants before the interviews started. Anonymity and confidentiality on the audio-recordings and the transcripts were secured by using subject identifier codes instead of names.
