Ethics approval and consent to participate
: Ethical clearance was obtained from the Human Research Ethics Committee at Charles Darwin University, Australia (HREC 2015–2445) and the Mozambican National Bioethics Committee and MoH (105/CNBS/2016). Approval was granted to obtain written and verbal consent from the international and local ethics committees. All participants were informed of the study purpose, the potential risks and benefits before obtaining written informed consent. Occasionally, verbal consent was received when participants were illiterate or did not feel comfortable signing the consent form due to fears surrounding confidentiality. All participants were given the right to refuse or withdraw at any point from the interview; no participants chose to withdraw.
