: The data used in this paper derive from a Department of Health commissioned audit that was undertaken in 2011–2015. Children and families who completed questionnaires as part of this audit consented for the information in the questionnaires to be linked to other information about them and for this to be used to underpin publications. This was agreed by the Caldicott Guardian at the Department of Health. No additional ethics were sought as this was judged an audit and not a research project.
