: The study was approved by a regional branch of the Danish Data Protection Agency (record no. 1-16-02-193-18). Registry-based studies do not require approval from ethics committees or informed consent from participants according to Danish regulation.
: Informed consent is not required from participants in registry-based, non-interventional cohort studies according to Danish regulation.

Mr. Madsen and Professor Sørensen has not received any personal fees, grants, travel grants, or teaching grants from companies.
