Ethics approval and consent to participate
: Ethical approvals for the study were obtained from the National Health Research & Ethics Committee, Solomon Islands (HRE002/16) and the James Cook University Human Research Ethics Committee, Australia (H6488). All research was performed in accordance with relevant guidelines and regulations of these research boards, and as stipulated in the approvals. Community meetings were held with all village residents, where the aims, the possible risks and potential benefits of the study were explained in Solomon Islands Pidgin. Participation in the study was voluntary and individual informed consent was then obtained from each adult participant > 18 yrears. For children between 13 and 18 years of age signed consent was obtained from both the minor and his/her parent or guardian; for children less than 13 years, signed consent was obtained from a parent or guardian.
