: The caregivers of study participants provided informed consent (or assent of child if aged 12–15 years) to participate in the study. Study documents were approved by the Burkina Faso Ministry of Health Research Ethics Committee (Deliberation No 2016-12-137), CNRFP Institutional Bioethics Committee (N<sup>o</sup>2016/000007/MS/SG/CNRFP/CIB), Durham University Department of Biosciences Ethics Committee (SBBS/EC/MIRA) and Liverpool School of Tropical Medicine Ethical Committee (Protocol number: 16/047).
