: This study was integrated into a larger clinical trial (the PRIMA study: NCT03916003) led by the Menzies School of Health Research, Darwin, Australia (Menzies), in collaboration with the icddr,b. Identification codes were assigned to each study participant to preserve confidentiality, and their professional roles are used for presenting study results. This study was approved by an addendum to the main research protocol of the PRIMA study by the ethical review committee of icddr,b, Bangladesh (#PR-19001, 7th September 2019) and the Human Research Ethics Committees (HRECs) of the Northern Territory, Australia (HREC 19–3288).Approvals for interviews and discussions were sought from relevant authorities. Study participants were provided with an information sheet explaining the objectives of the study and their confidentiality and all but IDI8, 15, 20, 21, 25 (who agreed to participate verbally) signed informed consent forms prior to participation.
