: The main study (i.e., phase IIb clinical trial in 2007) had obtained ethical approval from the Kenya Medical Research Institute National Ethical Committee, the Western Institutional Review Board in Seattle, the Central Oxford Research Ethics Committee, and the London School of Hygiene and Tropical medicine Ethics Committee. Written informed consent for the extension study and sample collections was obtained from parents or guardians of all the children with the use of approved consent forms in (Swahili or Giriama). The extended follow-up study received ethical clearance from the Scientific and Ethics Review Unit-Kenya Medical Research Institute (SERU/KEMRI/2887).
