Ethics approval and consent to participate
: The NDNS was conducted in accordance with the Declaration of Helsinki and the MRC Good Research Practice principles. All procedures involving human subjects were approved by the Oxfordshire A Research Ethics Committee and informed consent was obtained from all participants. Ethical permission for the cognitive think-aloud interviews was granted by the University of Southampton, Faculty of Medicine Ethics Committee [Ethics number: 30054]. Informed consent was obtained from parents using opt-out procedures and adolescents provided written informed assent prior to participating in the cognitive think-aloud interviews.
