: Ethical approval was granted for this study by the University of Calgary Conjoint Health Research Ethics Board (CHREB). The Ethics identification number is REB16–1488. All participants provided written and verbal consent prior to the start of the interview and participants were free to withdraw their consent at any point throughout the duration of the study.
