Ethics approval and consent to participate
: The Research and Ethics Committee of Muhimbili University of Health and Allied Sciences (MUHAS) reviewed and approved the study protocol in May 2017 (reference number 2017-05-24/AEC/Vol.XII/70). The District Executive Directors of Igunga and Nzega districts granted permission to conduct data collection. Then, an informed written consent was obtained from the participants and verbal consent was obtained from those who could not read and write after the local guide had introduced the research assistant and the procedures for research in each household including their rights to participate or withdraw from the study. .
