: Ethical and administrative approvals were gained from both the University of Manchester (<i>Ref: ethics/11454,</i> April 2012) and from the Saudi Ministry of Health General Directorate of Medical Research (<i>IRB No.: IRB-071</i>, June 2012). A signed informed consent was secured from the study participants before starting data collection. Consenting the study participants started with clear explanation of the overall study aim and objectives and methods of data collection and analysis. Participants with given the freedom to decide on their participation in the study. They were also allowed to withdraw from the study at any stage. They were ensured that their personal information and data will be anonymized and saved in a secured password-protected drive that is accessed by the chief investigator only.
: The consent form included a statement about the consent for publication with maintenance of confidentiality and anonymity of participants’ identity.
