Ethics approval and consent to participate
: Ethics approval was received from the Human Research Ethics Committee at the University of the Witwatersrand. Written permissions to conduct all data collection were provided by the Department of Health (national and the two provincial offices), the two selected District Health Offices and each of the 12 study clinic facilities. Participants gave signed consent to participate in the study. Data were stored in principal investigator’s password-protected computer. The study uses unique identifiers to refer to participants. In addition, names of study sites (except for the national DoH, which is the only one countrywide) have also been anonymised to protect their identity and confidentiality.
