Ethics approval and consent to participate
: The cost effectiveness analysis component of the study was classified as exempt from ethics review, as it only entailed retrospective evaluation of de-identified existing data. The interview component of the study was approved by the author’s institute, with ethics reference ECONCIA18–19-03. Full verbal consent was obtained for all patients interviewed, due to low literacy levels in the area rendering written consent inappropriate in many cases.
: Not applicable.
: The authors declare their position as employees and volunteer researchers for the organisation delivering the Right to Health Care programme. We do not believe that this affected the nature of the analysis conducted in any way.
