: Our study complies with the appropriate national research ethics process and R&D and governance processes. The study was externally reviewed by the National School for Public Health Research (SPHR) and the University of Sheffield research ethics committee (019420). Access to the PHPES data was granted by the Management Committee of the SPHR. These data are not publicly available, as they include individual-level data that allow for the identification of participants in our study, but are available from the corresponding author on reasonable request.
