: This study involves human participants. Ethical approval was obtained from University College London Research Ethics Committee Office for the Vice Provost Research (Project ID: 4063/002). The Ethiopian Public Health Association (EPHA) Internal Scientific and Ethical Review Committee (ISERC) provided an ethical waiver as they considered this part of the QI programme evaluation (EPHA/OG/5046/17). Permission to collect the data was also obtained from the regional health bureaus and the health facilities. Before each interview an information sheet containing information about the study prepared in the local languages was given to participants to read. Participants gave informed consent to participate in the study before taking part.
