: This trial was performed in accordance with ethical principles consistent with the Declaration of Helsinki, International Conference on Harmonisation (ICH) guidelines, International Society for Pharmacoepidemiology (ISPE) (2007) Guidelines for Good Pharmacoepidemiology Practices (GPP) and applicable legislation.
