Ethics approval and consent to participate
: Each phase in this study has its own ethics approval. The ethics approvals for the exploratory phase have been obtained from University of Melbourne Human Ethics Sub Committee (ID 1851129) and from the Ethics Committee, Faculty of Medicine, Public Health and Nursing, Universitas Gadjah Mada, Indonesia number (KE/0524/05/2018). A research permission letter from Yogyakarta government office has also been obtained to conduct this research in public primary care clinics (Number 074/5770/Kesbangpol/2018).Ethics approval for the development phase has also been obtained from the University of Melbourne Human Ethics Sub Committee (ID: 1853074.1). Ethics approval for the pilot phase will be sought pending on the results of the preceding two phases.
