: For the clinical surveillance system written informed consent was obtained before initiation of any study procedures. Protocols were approved by ethical committees of the Ifakara Health Research and Development Center (IHRDC) and later on the Ifakara Health Institute (IHI) and thereafter by the National Medical Research Co-coordinating Committees through the Tanzanian Commission of Science and Technology. With regard to data for the demographic surveillance system, verbal consent to participate was sought from the head of every household, and protocols were approved by the ethical committees of IHRDC and the National Medical Research Co-coordinating Committees through the Tanzanian Commission of Science and Technology.
