: This study is based on data from a randomized controlled trial, approved by the Regional Ethics Appeals Board in Stockholm, Karolinska Institutet, Sweden (ExternalRef removed), 2009/1848–31/2. The trial was registered in Current Controlled Trials (ISRCTN92249294). All study participant gave their written informed concent to participate in the trial.
