: Our study procedures were approved by the Medical Ethical Committee (METc) of the VU Medical Centre (2012/414). It was decided that the law regarding research on human subjects (Wet Mensgebonden Onderzoek, WMO) did not apply to our observational study. Although this meant that informed consent was not obligatory, patients were asked for their informed consent which was registered by the treating physician.
