Ethics approval and consent to participate
: We obtained ethics approval from the Faculty of Public Health, Universitas Indonesia (323/H2.F10/PPM.00.02/2016) and the Human Research Ethics Committee of the University of Sydney, Australia (Protocol number: 2015/115). We also secured a research clearance from the Ministry of Internal Affairs at the central level as well as the Office of National Unity and Community Protection at the provincial and district levels. We took written informed consent from all respondents before the interview. All individuals included in this research are 18 years or older.
