Ethics approval and consent to participate
: The ethics committee approval for this study was obtained from the University Hospitals Leuven Medical Ethics Committee in May 2013. All general practitioners provided informed consent. Individual informed consent from the patient could not be sought. Reminders showed up unexpectedly during practice when patient data deviated from evidence-based targets. It was not practical to ask for informed consent at these moments. Furthermore, reminding the physician of the informed consent forms at every consultation might have biased the study results in favor of the intervention.
