: The University of Pennsylvania Institutional Review Board (IRB) served as the coordinating center for this study and the Madigan Army Medical Center (MAMC) IRB provided review and oversight for the coordinating center and each of the three study performance sites, in compliance with all applicable federal regulations governing the protection of human subjects. The MAMC IRB reviewed and approved the University of Pennsylvania protocol (MAMC protocol # 214055) on April 9, 2014; the Evans Army Community Hospital protocol (MAMC protocol # 214056) on April 9, 2014; the William Beaumont Medical Center protocol (MAMC protocol # 215025) on January 27, 2015; and the Blanchfield Army Community Hospital protocol (MAMC protocol # 215045) on April 16, 2015. All study staff, including investigators at military installations, have completed the requisite human subjects training preparatory to research. All participants provided informed consent.
