: This study was approved by the institutional review board at the University of Tokyo. Japan’s Ministry of Health, Labour and Welfare approved our research protocols and provided the data. Our masked tabular data and the research results are also disclosed after inspection by the ministry.

: As the published data is de-identified and only contains summary data of patients, it is impossible to identify any person. In addition, we took much care for patients with ultra-rare diseases, following the NDB policy of cell size suppression, which was based on the CMS cell size suppression policy.
