: This study was approved by the Swedish Ethical Review Authority in Stockholm, Sweden (Dnr 2020 − 00490). Prior to the journal review, approval was obtained from the heads of operations of the participating organisations. The project received an approved ethics review without informed consent from patients whose records had been reviewed. The need for informed consent was waived by Swedish Ethical Review Authority in Stockholm, Sweden due to retrospective nature of the study. The motivation is that a record review is a common method for healthcare organisations to measure patient safety and quality of care. The approach in this research project does not differ from the journal review that normally takes place within healthcare organisations. People employed by the included organisations executed the journal reviews. The study ensured strict compliance with Swedish research ethics guidelines.
