: The study was reviewed and approved by the RUSVM Institutional Review Board (protocol 18–04-FL) and the Interim Ethics Review Committee of the Ministry of Health, St. Kitts and Nevis (approval code IERC-2019-11-032). Written informed consent is obtained from all participants prior to their enrollment in the study.
: In the informed consent document, participants provide consent to the following statement on publication, “Your information will be combined with information from other people taking part in the study.
