: Ethical clearance for this study was obtained from the Ethics Committee of the Faculty of Public Health, Universitas Indonesia. Administrative clearance was acquired from the Ministry of Internal Affairs, National Unity and Community Protection Office (Kesbanglinmas) at provincial and district levels, as well as local authorities from sub-district, village and hamlet level. Written informed consent was obtained from all respondents prior to the interview. All individuals included in this research are 18 years-old and above.
