: The protocol for the collection of field samples was approved by the Institutional Review Board of the US Naval Medical Research Center (Protocol NMRCD.2005.0005) and the National Institutes of Health of Peru (Protocol 009-2004) in compliance with all applicable Federal regulation governing the protection of human subjects and with the Declaration of Helsinki. All adult subjects provided written informed consent; all children 8–17 years old provided verbal assent to participate in the study. For children under the age of 17, informed consent was obtained from their parent or legal guardian.
