Ethics approval and consent to participate

: The study was approved by the Institutional Review Board at University of California, San Francisco (study #15-16067). Waiver of consent was granted for the collection of medical record data and automated telephone questionnaire data. Verbal consent was obtained for all subjects that completed the telephone questionnaire administered by a researcher. No individual patient data is published.
