: Ethical approval for the study was obtained from the Office of Research Ethics at the University of Toronto (Ref: 36569) and the Ethics Review Committee of the Xizang Centre for Disease Control and Prevention (Ref: 006). The information and consent form were translated via an interpreter and permission to be audio-recorded was collected. We ensured participant confidentiality by quoting participants, using pseudonyms.
