: Ethics approval was granted by the Flinders University Social and Behavioural Research Ethics Committee (SBREC) in May 2017 (Project No. 7601), and the trial was retrospectively registered with the Australian New Zealand Clinical Trials Registry (ANZCTR) (ExternalRef removed) (registration submitted 13 July 2017; registration recorded 25 July 2017; trial number ACTRN12617001079336), after the date of the first consent (6 July 2017), but before the date of the first intervention treatment session (20 July 2017). All participants were provided with a participation information sheet and consent form prior to interview, and informed written consent was obtained for study participation.
