: The cantonal ethics committee of Zurich confirmed to us in writing that the study does not fall within its area. The study was approved by the local ethics committee (Ethics Committee of the University of Ulm) and the internal ethics officer (University Medical Center Hamburg-Eppendorf). Furthermore, in all hospitals, the board of directors and the other relevant clinics’ internal departments (personal departments, departments of quality management and data protection commissioners) agreed with the study protocol. Participation in the study was voluntary and anonymous. The informed consent was obtained from all subjects, or if they were under 18 years, from a parent or a legal guardian.
