Ethics approval and consent to participate
: This study was conducted according to the guidelines laid down in the Declaration of Helsinki and all procedures involving human participants were approved by the National Committee of Ethics in Health in Guinea-Bissau (NoRef064/CNES/INASA/2013). Written informed consent, either by signature, or fingerprint if illiterate, was obtained from all participants. Due to the non-invasive nature of the study, adolescents aged 15–17 years were permitted to give informed consent without parental/legal guardian consent (approved by the National Committee of Ethics in Health in Guinea-Bissau).
