: Ethical clearance was obtained from Bahir Dar university institutional review board with the reference number PRCSVD/100/2013. Before data collection, a letter of permission was obtained from Bahir Dar zuria district health office and from the seven health centers included in the study. Moreover, before collecting clinical and anthropometric data, the aim of the study was explained to the parents/guardians of each child. Verbal and written consent was obtained from all parents/guardians of children involved in the study. The data provided by the participants were kept confidential and personal identifiers were excluded from the data collection form. We gave priority to participants’ convenience and we respected their rights as a participant during data collection. The authors confirm that all methods were carried out according to relevant guidelines and regulations (Helsinki declaration).
