: The protocol was peer-reviewed by the MUHAS Directorate of Research and Publications and granted ethical clearance by the MUHAS Senate Ethical Review Committee (Ref. No. 2014-11-07/AEC/Vol.IX/35). All respondents gave written informed consent. Respondents were identified by number. Data were secured in a locked room. The Health/Control group received a short health training, which all groups received and which addressed the ethical dilemma of not offering any incentives to the control group while expecting them to be part of the study.
