: The protocol was peer-reviewed by the MUHAS Directorate of Research and Publications and granted ethical clearance by the MUHAS Senate Ethical Review Committee (Ref. No. 2014-11-07/AEC/Vol.IX/35). All respondents gave written informed consent. Respondents were identified by number. Data were secured in a locked room. The Health/Control group received a short health training, which all groups received and which addressed the ethical dilemma of not offering any incentives to the control group while expecting them to be part of the study.
: All study participants signed a MUHAS-approved consent form after being informed about the study and their rights. The consent form made clear that participants would not be named or personally identified in any report.
