Ethics approval and consent to participate
: In both cohorts, all research participants or their surrogates provided written informed consent. Written informed consent was obtained from all participants who could read and write. For those who could not, the study team explained the consent to them and the participant used a thumb print as their signature. For unconscious patients, surrogate consent was obtained from the relatives or legal representative and later the patient would be reconsented at they recover. Ethical approval occurred from the Uganda National Council of Science and Technology (UNCST), Mulago Hospital Research and Ethics Committee, and the University of Minnesota.
