Ethics approval and consent to participate
: Ethical approval was obtained from the Institutional Review Board of the Institute of Medicine, Tribhuvan University. Approval was obtained from the Bhaktapur District Public Health Office to conduct research in the district. Information was provided on potential risk, discomfort and benefits to participants, and confidentiality, the right to refuse or withdraw, and right to information. Informed consent was obtained from the respondents (informed and written). For the respondents below 16 years of age parental consent was taken.
