: The study was carried out according to Ethical Guidelines for Medical and Health Research Involving Human Subjects published by the Ministry of Health, Labour and Welfare; the laws and regulatory requirements of Japan; and the ethical principles that have their origin in the Declaration of Helsinki (as revised in Tokyo 2004).
