: This investigation was in response to a public health emergency (anthrax outbreak) and was, therefore, determined to be non-research. The Office of the Associate Director for Science, CDC Center for Global Health, also determined that this activity was not human subject research, and its primary intent was public health practice or a disease control activity (specifically, epidemic or endemic disease control activity). We obtained verbal informed consent from case-patients during this investigation and other interviewed community members who were above 18 years. Community health workers referred case-patients identified in the community to Engari health centre for further management. The participants had to understand and follow the interview proceedings. The questionnaires were kept under lock and key to avoid disclosure of personal information of the respondents to members who were not part of the investigation.
