Ethics approval and consent to participate
: Data collection, processing, and analysis were approved by the institutional ethics committee (Ethics Commission of the Faculty of Medicine of the Technical University of Munich, protocol numbers 180/17 and 5573/12). The study was designed as a retrospective cohort study with a prospective validation cohort. The requirement for written consent was waived for the retrospective cohort and written consent was obtained for the prospective cohort. All procedures were carried out in accordance to pertinent laws and regulations.
